Caught in the act of breaking the law

Garbage bags filled with used medical gloves, some of which were found to be soiled, some even stained with blood, are scattered on the floor of a warehouse on the outskirts of Bangkok, Thailand. .

Next to it was a plastic bowl full of blue dye and some gloves. Thai officials said migrant workers were trying to “refresh” the gloves, when authorities raided the facility in late 2020.

Nowadays, in 2021 many of these warehouses continue to operate in Thailand, trying to cash in on the high demand for medical gloves made of nitrile material – demand has skyrocketed due to Covid-19 pandemic.

And then people pack millions of these substandard gloves for export to the US and many other countries around the world amid a serious global shortage of such medical items.

A months-long investigation by CNN (USA) found that tens of millions of fake or used nitrile gloves have arrived in the United States, according to import records and the distributors who purchased the number. gloves now. And that is just the tip of the iceberg. US and Thai authorities also conducted a parallel criminal investigation into the incident.

Douglas Stein – one of the experts in the field, said that nitrile gloves are “now the most dangerous item on Earth”.

“There is a huge amount of bad product flowing into the United States, an endless stream of dirty, used, and substandard gloves,” says Stein. It seems that federal officials are only now beginning to understand the enormity of the problem.”

Despite the great risk to frontline healthcare workers and patients alike, US officials are still struggling to crack down on this illegal trade, in part due to regulations governing the import of protective gear. has been temporarily disabled due to the complicated development of the epidemic and this has continued to this day.

In February and March 2021, an American company alerted two US federal agencies (Customs and Drug Administration) that it had received from a company in Thailand shipments full of gloves. Hands are below standard and stick to the ground.

Despite that, the Thai company still managed to ship tens of millions of gloves in the following months, with some shipments arriving as recently as July 2021.

Rising demand in the US for medical gloves

In early 2020, the demand for personal protective equipment (PPE) “increased through the ceiling” as the Covid-19 pandemic spread around the world. And the price of nitrile gloves is still high. Nitriel medical gloves are commonly used by doctors and medical staff to examine patients. The FDA (US Food and Drug Administration) has banned the use of powdered latex in the medical industry, and gloves made from low-quality vinyl are more common in industry and food processing.

The aforementioned gloves are mainly manufactured in Southeast Asia, based on limited supply from factories specializing in the production of natural rubber. Supply cannot increase overnight and production for major brands is ordered years in advance.

As governments and health systems rush to find medical gloves, dozens of underground companies are looking to quickly profit from this opportunity.

In late 2020, Miami businessman Tarek Kirschen ordered about 2 million gloves from a Thailand-based company called Paddy the Room. The businessman then sold the gloves to a distributor in the US.

Tarek Kirschen recalls: “We started getting calls from very frustrated customers. Turns out, these are all used gloves. They are washed for recycling. Some are still very dirty. Some even had blood stains. Some still have the date of manufacture 2 years ago… I can’t believe my eyes anymore.”

Entrepreneur Kirschen said he returned the money to the customer, buried the gloves, and alerted the FDA in February 2021.

According to Kirschen, none of the gloves he ordered were used in medicine. But a CNN analysis of import records shows that other US distributors received nearly 200 million gloves from Paddy the Room during the pandemic. It is not clear what happened next to the gloves after being imported into the US.

CNN tries to contact all importers. The majority did not respond but 2 importers said the shipments were substandard and the gloves weren’t even made of nitrile. One of the companies, Uweport, told CNN it could not resell the gloves it had purchased to medical companies as planned. Instead, they have to sell these gloves at a lower price to suppliers who specialize in supplying gloves to food processing factories, hotels and restaurants in the US.

The other company, US Liberty LLC, had a similar experience with Paddy the Room. They said they received the goods from another company in Southeast Asia. They said, “gloves had holes in them, had stains on them, and had different colors.”

Got caught here, moved to another place

Meanwhile, the Food and Drug Administration of Thailand (Thai FDA) has struggled to combat the fraudulent trade of nitrile gloves.

When Thai FDA employees raided Paddy the Room’s facility in December 2020, they found piles of trash bags filled with loose gloves of different colors, materials, and qualities. Workers at this warehouse are shoving old gloves into boxes of new fake Sri Trang. Sri Trang is a legal and well-known trademark of a glove manufacturer in Thailand. The brand said it does not do business with Paddy the Room.

Thai FDA arrested the warehouse owners but failed to prosecute them. The raid also did not cause Paddy the Room to be closed. Months later, Thai FDA Deputy Secretary General, Supattra Boonserm, said her agency had conducted another raid on a similar facility.

“They just moved to a new location, a new warehouse. Why is that? That’s because the demand for gloves is still high. There are still customers who want to buy.”

Thai FDA says it has conducted at least 10 raids in recent months and seized counterfeit or used gloves that were repackaged as new. During several such inspections, Thai authorities discovered workers in these facilities were washing gloves and then re-staining them.

Ms. Boonserm suspects these gloves were purchased from China or Indonesia and then exported to Thailand to be washed, dried, and repackaged. “Simply put, this is cheating.”. Therefore, confidence in medical gloves imported from countries like Thailand is declining. Instead, countries like Vietnam, with prestigious glove brands like VGlove, are increasingly gaining the trust of US importers.

In particular, it is necessary to promote the dissemination of fast, correct and sufficient information on both sides of Covid-19 prevention and control”, VRG Chairman Tran Ngoc Thuan requested.

TTO – The Ministry of Health said that the unit has just issued an urgent dispatch instructing the use of personal protective equipment in the prevention of the COVID-19 epidemic. In it, there are instructions for medical staff to take COVID-19 test samples ‘no

On June 25, the Ministry of Health said that the unit has just issued an express dispatch instructing the use of personal protective equipment in the prevention of the COVID-19 epidemic.

The official dispatch stated: in the opinion of the World Health Organization office in Vietnam, based on risk assessment and mode of transmission of the SARS-CoV-2 virus, the use of personal protective equipment in Vietnam is currently is not entirely reasonable. Especially in hot weather conditions and the increased demand for testing has affected the health of samplers.

Accordingly, in order to ensure the health of medical staff, ensure the quality of patient samples, and prevent epidemics, especially in hot weather, the Ministry of Health temporarily guides the use of medical equipment. Personal protection when taking patient samples as follows:

1. Based on local resources, the testing facility provides and instructs staff to take samples for COVID-19 monitoring, who can use a loose suit of suitable sizes including a long-sleeved medical gown Full body coverage from neck to knees, fixed laces at the nape of the neck and around the neck, hood, removable shoe covers. It is not necessary to wear full body protection.

2. The person taking the sample needs to follow the correct procedure for using a mask to ensure a snug fit to the face. Do not use 2 layers of masks including a medical mask inside a mask with a high filtering effect (such as an N95 mask or equivalent).

3. During the sampling process, you can use 1 pair of medical gloves (without talcum powder) and disinfect the gloves with a quick disinfectant solution after each sampling, change gloves after disinfecting up to 6 times. or when gloves are damaged to replace wearing 2 pairs of gloves and changing gloves after each sampling.

4. Staff taking samples, medical staff participating in sampling activities absolutely follow the process of wearing and taking off personal protective equipment; During the sampling process, do not perform other activities (such as eating, drinking).

The Ministry of Health said the above 4 requirements are the minimum requirements when taking samples for testing for SARS-CoV-2. Previously, many health workers fainted from exhaustion after hours of wearing protective gear to take COVID-19 test samples in the heat of summer.

Garbage related to COVID-19 can also become a source of dangerous pathogens if not collected and disposed of properly.

During the complicated development of the COVID-19 pandemic, the volume of medical waste has increased significantly. The increased amount of waste during this time easily becomes a source of infectious pathogens as dangerous as F0, F1. The disposal and treatment of waste of sick people and people living in environments at risk of COVID-19 infection should receive more attention.

The types of waste associated with COVID-19 are usually masks, disinfectant bottles, blood-stained tissues, PPE kits, gloves, and household waste. These items can be viewed as infectious waste. This type of waste is suspected to contain pathogens such as viruses, bacteria, fungi and parasites, capable of making healthy people sick.

This waste also includes medical materials and devices used to diagnose, detect and treat COVID-19. In essence, they have been contaminated with blood, tissues, body fluids, organs, needles, thermometers, toilet paper, gauze, urine bags, and other medical devices or materials that have come into contact with the fluid. secretions of infected patients.

According to one study, the expected amount of hazardous waste generated per day from an infected person can reach a maximum of 3.4kg. The sudden increase in the amount of waste generated by patients infected with COVID-19 has put enormous pressure on the waste management industry in every part of the world, especially in the epidemic areas.

Infectious waste can cause environmental and health problems if not stored, transported or handled properly. According to one study, the COVID-19 virus can remain active on surfaces such as plastic, metal and glass for up to 9 days, on serum samples for 11-12 days, 17-31 days in feces and 13-29 days in respiratory secretions.

To ensure the safety of those around you, here are some things to keep in mind when disposing of COVID-19 related waste:

If there is a confirmed or suspected case of COVID-19 in the household, members should be aware that patient waste can also be a source of the disease. Therefore, it is necessary to handle them carefully. Patient waste should be collected early and not left outdoors.

Medical waste should be kept separate from other household waste. Waste containers must be tightly closed after storing garbage. To avoid direct contact with waste, the bag must be sealed before the waste exceeds 70% of the bag’s capacity. This trash bin or bag/container needs to be kept out of reach of scavenger animals such as crows or rats.

One of the most important things is that people who dispose of garbage need to wash their hands after coming into contact with trash cans/bags. During treatment, infectious waste should be placed in separate bins or boxes, possibly with prominent colors for easy identification. In addition, leftovers and food packaging used by COVID-19 patients can still be considered waste with a certain risk of infection.

Quang Tri Provincial General Hospital confirmed that from the beginning of March up to now, this unit is always in a state of restlessness of medical gloves. And by this point, the whole hospital had run out of sterile gloves.
On April 1, the Quang Tri General Hospital (BV) confirmed that from the beginning of March until now, this unit is always in a state of being able to take care of all medical gloves and up to this point the whole hospital has run out of gloves sterile, only 500 pairs of loose gloves left. Not only at Quang Tri General Hospital but many other medical facilities in the area also fell into a situation of “crisis” and lack of medical gloves.
The Department of Health of Quang Tri province also confirmed that the supply of medical gloves for medical facilities in the province used for the year 2020-2021 is done by 3 contractors, the Provincial International Pharmaceutical Company Limited. Dak Lak, Lan Tien Pharmaceutical Company Limited in Quang Tri Province and Hoang Duc Pharmaceutical Company Limited in Quang Tri Province.

However, due to the Covid-19 translation leading to a scarce commodity, these 3 companies applied to cancel the contract and received the consent of the People’s Committee of Quang Tri province; At the same time, the Department of Health was assigned to work out a plan to adjust the bidding package for re-bidding. While waiting for the new package, the Department of Health of Quang Tri suggests the above companies continue to try to provide medical gloves for the hospitals. Therefore, companies have tried to source and supply to medical facilities but in a drip-free fashion.
Mr. Ha Lam Chi, Deputy Director of the Department of Health of Quang Tri province, said that following the instructions of the Provincial People’s Committee, the Department has implemented the bidding according to regulations, next week will open the bid.

In a study conducted by JO SAWYER and ALLAN BENNETT, scientists questioned the difference between nitrile examination gloves and natural latex gloves under actual conditions of use.

Abstract

An increase in the occurrence of latex allergy has been concurrent with the increasing use of latex gloves by laboratory and healthcare workers. In recent years nitrile gloves have been used to replace latex gloves to prevent latex allergy. Nitrile gloves offer a comparable level of protection against chemical and biological agents and are more puncture resistant. However, if manual dexterity is compromised by nitrile gloves to a greater degree than latex then this may increase the risk of sharps injuries. The Purdue pegboard test, which measures both gross and fine finger dexterity, was used to test the dexterity levels of two glove types used at HPA CEPR; Kimberly-Clark SafeSkin nitrile and latex laboratory gloves. There was a statistically significant 8.6% increase in fine finger dexterity provided by latex compared with nitrile SafeSkin laboratory gloves but no difference in gross dexterity between the glove types. There was no significant relationship between glove dexterity and age or gender. The selection of glove size was influenced by the digit length of participants. Moreover, those with longer, thinner fingers appeared to have an advantage when using nitrile SafeSkin gloves. The level of dexterity provided by latex and nitrile SafeSkin gloves for tasks on a gross dexterity level are comparable and health workers will benefit from the non-allergenic properties of nitrile. For tasks requiring fine finger dexterity nitrile SafeSkin gloves may impede dexterity. Despite this, the degree of restriction appears to have a negligible impact on safety in this study when compared with the risk of latex sensitization and subsequent allergy. In addition to glove material, working practices must also take into account glove size, fit, grip and thickness, as these factors can all influence dexterity.

Introduction

The dramatic increase in the use of latex gloves began in the 1980s following a greater awareness of HIV/AIDS. The incidence of natural rubber latex (NRL) allergy became more common as a result (Turjanmaa, 1995; Edlich et al., 2003; Ranta and Ownby, 2004) and alternative glove materials were sought. A range of glove materials were tested for barrier effectiveness against bloodborne pathogens (Rego and Roley, 1999), simulated clinical use (Korniewicz et al., 2002), puncture resistance (Fisher et al., 1999; Patel et al., 2004) and durability (Kerr et al., 2004), and in all these tests vinyl and copolymer gloves gave a worse performance than latex gloves. However, nitrile gloves gave a performance comparable with latex. Many hospitals and laboratories are now replacing latex with nitrile gloves, and several studies have documented a reduction in the incidence of latex allergy when implementing this policy (Allmers et al., 2002; Hunt et al., 2002). It is envisaged that more institutions will switch from using latex to nitrile gloves in the future.

Nitrile gloves have different physical properties to latex (Fisher et al., 1999; Jackson et al., 1999; Patel et al., 2004), which could affect the dexterity of the user. Recent research has shown that dexterity can be adversely affected by the use of latex (Neiburger, 1992) and chemical protective gloves (Bensel, 1993), the type of containment equipment used, and also by using the wrong glove size. There has been suggestion that nitrile gloves may produce finger and hand fatigue over time due to the inflexibility of the glove material (Morris, 1994; Korniewicz et al., 2004).

A reduction in dexterity caused by wearing certain types of laboratory gloves could increase the risk of injury from sharps or chemical spills for health and laboratory workers. Therefore, the aim of this study is to use the Purdue pegboard test to quantify the dexterity levels provided by the type of latex and nitrile laboratory gloves used at the Health Protection Agency, Centre for Emergency Preparedness and Response (HPA CEPR). The influence of age, gender and digit size will also be investigated. Although this study relates to workers at HPA CEPR, it is anticipated that laboratory and healthcare workers on a wider scale will find the results of value.

As a result, after many trials in aspects including Gross dexterity, Fine finger dexterity, Thumb and finger dimensions, regarding the gloves specification, age and gender of testers, scientists have come to conclusions.

Conclusions

Nitrile SafeSkin gloves offered an alternative to latex that will not promote latex allergy at the HPA CEPR, but it was not known whether the levels of dexterity were comparable. Both glove types were tested using the Purdue pegboard to measure dexterity. Despite being a thicker material, SafeSkin latex gloves provided an 8.6% higher level of fine finger dexterity compared with SafeSkin nitrile gloves. There was no significant difference between the gross dexterity provided by latex and nitrile SafeSkin gloves and no relationship between age and gender for the dexterity provided by any of the gloves tested.

Participants with shorter fingers appeared to have reduced dexterity with the nitrile SafeSkin gloves, but there was no reduction in dexterity with the latex gloves. This could be an artefact of the lack of elasticity in nitrile SafeSkin gloves compared with latex. Nitrile SafeSkin gloves also were perceived by some participants to be a tighter fit than latex. Perhaps the manufacturers of SafeSkin need to reassess nitrile glove size to provide an optimum fit.

The majority of participants both preferred and use latex SafeSkin gloves. This could be a consequence of familiarity as many participants had not used nitrile gloves before. However, although in this study the perception of glove users that nitrile gloves seriously reduces dexterity is unfounded, tasks requiring fine finger dexterity should be given extra time if necessary.

Nitrile SafeSkin gloves should be considered a non-allergenic alternative to latex SafeSkin gloves that does not seriously compromise dexterity. This information should not only be valuable to the HPA CEPR but also will benefit laboratory and healthcare workers.

and raised some questions over the current types of gloves selling in the market

Further work

Only two types of gloves from one manufacturer were assessed in this study. Many types of gloves are on the market, and the majority advertised the high levels of dexterity provided. Despite this, no data were provided to back these claims. Recommendations for further work include the assessment of dexterity levels provided by other gloves from a range of manufacturers. A dexterity test based directly on common laboratory techniques may provide further data on the suitability of different types of gloves for specific tasks. Additionally, latex and nitrile gloves should be compared and tested for longer periods of time to mimic and assess the effects of hand and digit fatigue that can occur during laboratory work.

Details of the research results can be found in the article below.

The police of Cu Chi Ho Chi Minh City has just received the denunciation of Ms. Nguyen Ngoc Thuy (living in Cu Chi district, Ho Chi Minh City; who is authorized by Hieu Hien Tay Ninh Co., Ltd.) to denounce the Thai Binh Duong company “doing business. deceit, violate the contract without compensation as agreed and have signs of appropriating the deposit amount “up to billions of dong ?.

Accordingly, on July 8, 2020, Hieu Hien Tay Ninh Co., Ltd. signed a “Medical glove sale and purchase contract” with Thai Binh Duong Company (Mr. Tran Ngoc Quan as director; address at Xuan Street. Thoi Thuong 21, Xuan Thoi Thuong commune, Hoc Mon district, HCMC) to buy 20,000 boxes of Nitrile Vglove artificial medical gloves with a total value of 33 billion VND.

According to the signed contract, Pacific Company agreed to sell medical gloves according to the specifications and standards of VRG Khai Hoan JSC which are Nitrile VGlove synthetic rubber medical gloves which have been approved by the Ministry of Health, fully certified and standard.

According to the agreement, Hieu Hien Tay Ninh Company makes a deposit of 2 billion VND for Thai Binh Duong company immediately after signing the contract. The product delivery deadline is before 17:00 on July 9, 2020. In case Hieu Hien Tay Ninh Company does not receive the goods or does not pay the balance in full, the deposit will be lost, otherwise, if Thai Binh Duong does not deliver the goods, it must refund the deposit and compensate 100% of the value.

The two sides agree on a location for the delivery of the shipment at Khai Hoan factory warehouse (Cau Sat hamlet, Lai Hung commune, Bau Bang district, Binh Duong province). Hieu Hien Tay Ninh Company then transferred the 2 billion VND deposit amount to Thai Binh Duong Company.

At the appointment to deliver the shipment, Hieu Hien Tay Ninh Company sent the car to the place to receive the goods, but Thai Binh Duong did not deliver the shipment as signed.

After that, Hieu Hien Tay Ninh Company came to the headquarters of Thai Binh Duong Company to request to return the deposit and pay compensation as agreed in the contract. However, Pacific company did not agree and tried to withdraw.

It was not until July 18, after many times that the company requested to return the above deposit, did Thai Binh Duong transfer 1 billion VND to Hieu Hien Tay Ninh Company, the remaining amount would not be paid.

Realizing that the Thai Binh Duong company showed signs of deliberately “appropriating” the deposit amount, there were signs of fraudulence to appropriate property, so Hieu Hien Tay Ninh Company made a complaint to the police.

To clarify the above contents, a reporter of the Law Protection Newspaper contacted Thai Binh Duong Company. At the meeting, the representative of this company admitted that he had violated the contract signed with the company Hieu Hien Tay Ninh, but only agreed to settle it according to civil lawsuit procedures.

With the question, what is the basis for Thai Binh Duong company to sign a contract to sell medical gloves above with Hieu Hien Tay Ninh Company? Does Thai Binh Duong Company sign a cooperation agreement with the Khai Hoan medical glove factory? Representatives of this company were unable to provide them, but only offered an economic cooperation contract with another partner.

It is known that the police of Cu Chi district, Ho Chi Minh City has invited the two sides to work under the denunciation.
